Project »COMFORTAGE«: Prediction, Monitoring and Personalized Recommendations for Prevention and Alleviation of Dementia and Frailty

Demographic change is leading to an increased incidence of age-related diseases such as dementia and frailty. Early diagnosis and targeted treatment of these diseases can significantly improve the quality of life of those affected. However, neither the medical expertise nor comprehensive data on individuals is generally available to a sufficient extent.

Individual Interventions and General Strategies

In the project »COMFORTAGE«, we are working together with numerous European partners from research, healthcare and industry on methodological and software tools for dealing with dementia and frailty. These solutions are used to generate case-specific recommendations and develop general approaches for the prevention and alleviation of age-related diseases based on digitized medical knowledge and the analysis of patient data. This enables a significantly higher quality of life for those affected. We contribute our expertise in such planning and analysis tasks, in particular with the development of decision support systems.

Health Planning With Artificial Intelligence and Decision Support

The digital environment created in »COMFORTAGE« uses a digital health reference model to describe the cases. Based on this, available data is merged and additional patient data collected using person-centered technologies such as wearable sensors and digital learning games is integrated.

This case data is analyzed using innovative artificial intelligence (AI) methods and possible individual prevention and intervention measures are identified. Based on these analysis results, treating physicians plan the healthcare of their patients with the help of decision support tools. The integrated case data and AI results are also used by healthcare professionals to continuously develop general prevention and intervention strategies. These strategies in turn serve as the basis for continuously improved individual case planning.


Funding and Duration

The project is funded by the Horizon Europe Framework Program »Computational Models for New Patient Stratification Strategies«. The funding starts in January 2024 and runs until the end of 2027.
